申报进口化妆品许可批件（备案）需提供的资料：
一.优先提供的资料：
1.经公证的在华申报责任单位授权书和声明书（审核，通过审核后在申报时提供已经备案的行政许可在华申报责任单位授权书复印件并加盖公章）；
2.用户名申请表；
3.委托书（委托办事）
4.提供企业及产品信息采集表内各项内容；
5.产品配方（要求全成分标注，复合配方标注复合比例，并提供使用目的）；
6.申请育发、健美、美乳类产品的，应提交功效成份及其使用依据的科学文献资料；
7.检测申请表；
8.送检所需样品（要求同一批次且未开封，不少于17套）；
9.产品使用说明书（中文）；
10.营业执照副本复印件加盖公章。
